Meta-analysis comparing outcomes in patients undergoing transcatheter aortic valve implantation with versus without percutaneous coronary intervention

Abstract Background Patients having transcatheter aortic valve implantation (TAVI) routinely undergo coronary angiography before the procedure to define anatomy and evaluate extend of artery disease (CAD). Whether percutaneous intervention (PCI) prior/concomitant with TAVI confers any additional clinical benefit in patients CAD remains unclear. Purpose To compare outcomes PCI prior significant severe stenosis. Methods We systematically searched PubMed, Embase Cochrane databases, November 2021, for both retrospective prospective studies comparing versus alone. Random-effects meta-analysis was performed. Results Eleven were included which one a randomized trial. A total 2530 included, providing 145 pooled death events (64 81 only). In terms 30-day outcomes, our analysis revealed similar all-cause mortality (pooled OR, 1.24; 95% CI [0.80, 1.93], P=0.34; I2=27% - Figure), cardiovascular 1.44; [0.56, 3.75], P=0.45; I2=57%) stroke 1.07; [0.53, 2.13], P=0.86; I2=0%). However, higher rate myocardial infarction 4.28; [1.56, 11.69], P<0.01; I2=0%) major bleeding 1.40; [1.02, P=0.04; group. 1-year trend lower only group 1.37; [0.98, 1.91], P=0.06; I2=0%), 1.15; [0.70, 1.89], P=0.59; I2=6%) 1.62; [0.95, 2.76], P=0.07; Conclusion Our data suggests that stenosis concomitant grants no advantage.
